Albert Road is a specialist neurological care service providing residential support for adults following a brain injury or with a neurological condition. Our progression model of care offers transitional / long term residential care living in a well-equipped residential setting.
Supporting people through post-acute rehabilitation, our goal is to promote and maintain optimum independence and choice through a personalised support plan. The personalised support plan or GAS (goal attainment scaling) is both written and reviewed in consultation with the individual person to help maintain their optimum goals through the management of their specific needs.
With a focus on cognitive impairment we have individual support guidelines to cover functions such as attention, memory, information processing and executive skills. We also work with SASNOS, a tool used to establish areas requiring more support such as relationships, social skills and behaviour.
Facilities
Having recently undergone an extensive refurbishment, Albert Road offers 7 en-suite bedrooms, situated over 4 floors with lift access to each. Communal spaces include a spacious activities room, two lounges, a dining room and two kitchens, one of which is for the people supported to prepare and cook their own meals - with support.
Outside, Albert Road has a large garden with a wheelchair accessible decked area, raised flowerbeds and a large gazebo which is regularly used to provide safe family visits. There is also on-site parking.
