Within both our residential and supported living environments, National Care Group delivers quality outcomes for the people we support and promotes independence and choice through our care plans. Another word for an outcome is a target or a goal. Each and every one of us sets outcomes for ourselves on a daily basis. Here at National Care Group, we support individuals with learning disabilities, mental health needs, autism spectrum conditions, acquired brain injuries and physical disabilities to achieve their own personal outcomes. It’s our mission to help the people we support fulfil their potential, develop new skills and live as independently as possible.
Encouraging every individual to aim high and to live the life they want
One of National Care Group’s principal objectives is to inspire and empower. We encourage every individual to aim high and to live life the way they desire – no goal is too small and no dream is beyond reach. We know outcomes are unique to each individual and could be anything from participating in local community events, to learning to cook a meal, to enrolling in college or travelling solo.
Our aim is to help the people we support identify a goal and then reach it. We take the time to sit down with each individual to discuss their aspirations and dreams, what they would like to learn and ultimately what they would like to do with their life. Our support teams then work with the individual to break down the overall outcome into small achievable steps, creating a clear development plan to work towards. These plans are reviewed at regular intervals and where appropriate, can lead to individuals moving on to lower acuity services. We then support people during the transition period from service to service; helping them make the step forwards from residential care to supported living and when suitable, to outreach support.
