In both our residential and supported living environments, National Care Group strives to deliver quality outcomes for the People We Support, with independence and choice guiding all our care plans. Other words for outcomes are targets or goals. Each and every one of us set outcomes for ourselves on a daily basis. Here at National Care Group, we support people with learning disabilities, mental health needs, autism spectrum conditions, acquired brain injuries, and physical disabilities to achieve their own personal outcomes. It’s our mission to help those we support to fulfil their potential, develop new skills and live as independently as possible.
Encouraging every individual person to aim high and to live the life they want
One of National Care Group’s principal objectives is to inspire and empower. We encourage every individual person to aim high and to live life the way they desire – no goal is too small and no dream is beyond reach. We know outcomes are unique to each individual person and could be anything from participating in local community events to learning to cook a meal, enrolling in college, or traveling on their own.
Our aim is to help the People We Support to identify a goal and then reach it. We take the time to sit down with each individual person to discuss their aspirations and dreams, what they would like to learn, and ultimately what they would like to do with their life. Our support teams then work with them to break down the overall outcome into small achievable steps, creating a clear development plan to work towards. These plans are reviewed at regular intervals and where appropriate, can lead to people moving on to lower acuity services. We then support people during the transition period from service to service; helping them make the step forwards from residential care to supported living and when suitable, to outreach support.
