Your next steps

  1. Talk to us. Call our friendly referrals team, complete a form, or email us, and we’ll start gathering all the information we need to start the process.

  2. One-to-one assessment. We’ll arrange a one-on-one appointment to spend time with you, your family, and any other important people. We will learn about your current situation, support and accommodation needs, and goals and ambitions.

  3. Explore options. We will arrange for you to visit our recommended home, meet the team who will be supporting you so you can ask questions and even meet some of the other people we support.

  4. A personalised support plan. We’ll produce a proposal detailing the arrangements and the support hours you require and keep you up to date every step of the way as we work with local authority funders to reach an agreement on your funding.

  5. Transition support. Once the funding is agreed upon, we’ll set a start date with you and begin the arrangements. Our local team will ensure everything runs smoothly.
