Transformational Journey Back to
Supported Living

Affinity Supporting People has empowered SC to lead a happier and healthier life

The passion and dedication of colleagues at Affinity Supporting People East Lancashire have empowered SC, a person we support who has experienced a very challenging few years, to transition back to supported living and lead a happier and healthier life.

Introducing SC

SC had experienced a challenging few years before moving into National Care Group’s Affinity Supporting People East Lancashire. His previous home had closed, and then he spent significant time in a hospital facility.

His learning disabilities nurse explained that the long-term hospital stays had badly affected SC. He refused to leave the house due to his anxiety, didn’t want to eat meals, and struggled with the sensory impact of cutting or brushing his hair.

He also struggled with personal hygiene, which led to a dental infection. Due to the severe pain, this led to behavioural challenges.

Empowering SC to achieve goals

Colleagues continued to support SC with compassion and dedication through some extremely challenging times.

Working together, they created a detailed, tailored support plan to help him develop independence, skills, and coping mechanisms. The ultimate goal was to empower SC to transition back into supported living.

Remarkable achievements by both colleagues and SC include:

  • Colleagues adapted records and monitoring to ensure that changing health needs were monitored

  • The service manager successfully supported SC back into the community after more than three combined years of him not leaving the building

  • Colleagues implemented visual aids to improve his eating habits, activity diaries, and now and next boards to decrease his overload and anxiety

  • The service manager and team leader supported colleagues in enhancing their confidence and skills to support SC

  • Colleagues successfully introduced an iPad to SC and supported him to engage in other activities. In his previous provisions, SC would not tolerate this stimulation

  • Colleagues supported SC to have a haircut after more than four years. It is the first time in SC’s life that his parents haven’t needed to be involved

  • Colleagues arranged and supported SC to attend a planned hospital admission for a full health check and receive the much-needed dental work

  • SC has gained new skills and developed stimulation that he never had before, thanks to colleagues.

Living a happy, independent life

As a result of the dedicated support from colleagues at Affinity Supporting People East Lancashire, SC’s support plan has now been reduced.

The intensive support team, social worker, and learning disabilities teams are delighted with SC’s progression and have discharged him. It is the first time in over 15 years that SC hasn’t required a social worker.

“The team deserves recognition for their achievements. This success story could not have been achieved without living and breathing the National Care Group values: passion, empowerment, respect, and collaboration. SC’s transformational journey back to supported living is a glowing example of what can be achieved when a team works together.”
