Outreach support
Our outreach support services assist individuals to live as independently as possible in their own homes and communities.
Domiciliary support workers will visit at a time and frequency agreed through the assessment process. This enables people to control the service they receive and to live life on their own terms.
We can develop a plan that ensures people are able to go shopping, attend medical appointments, participate in sports and fitness activities, do their cooking, cleaning, and banking, or attend social groups to maintain a busy and active lifestyle.
Whatever a person’s need, a plan will be tailored to support them.
