Chosen Care Supported Living offers people with learning disabilities, mental health support needs and autistic people support in their own home to empower them to live as independently as possible.

The support
Our supported living services deliver high-quality, person-centred support and enablement in a person's own home. Chosen Care's pathway focuses on ensuring continuity of support, collaborating with individuals to achieve their goals and to meet their choices. Our team is trained to specialise in the diverse needs of those we support, such as profound and multiple learning disabilities (PMLD) and the use of various aids and hoists.
The properties
We currently have 12 supported living services in the Chosen Care portfolio and support four people through outreach.
The location
The suburban areas of Gloucester provide the people we support a great base to call home. They benefit from a wide range of amenities on their doorsteps, as well as many green spaces and good transport links into Gloucester centre.
