The passion and dedication of colleagues at Affinity Supporting People East Lancashire have empowered SC, a person we support who has experienced a very challenging few years, to transition back to supported living and lead a happier and healthier life.
SC had experienced a challenging few years before moving into National Care Group’s Affinity Supporting People East Lancashire. His previous home had closed, and then he spent significant time in a hospital facility.
His learning disabilities nurse explained that the long-term hospital stays had badly affected SC. He refused to leave the house due to his anxiety, didn’t want to eat meals, and struggled with the sensory impact of cutting or brushing his hair.
He also struggled with personal hygiene, which led to a dental infection. Due to the severe pain, this led to behavioural challenges.
Colleagues continued to support SC with compassion and dedication through some extremely challenging times.
Working together, they created a detailed, tailored support plan to help him develop independence, skills, and coping mechanisms. The ultimate goal was to empower SC to transition back into supported living.

As a result of the dedicated support from colleagues at Affinity Supporting People East Lancashire, SC’s support plan has now been reduced.
The intensive support team, social worker, and learning disabilities teams are delighted with SC’s progression and have discharged him. It is the first time in over 15 years that SC hasn’t required a social worker.
