National Care Group works closely with commissioners, local authorities, and other organisations to provide opportunities for people to progress, develop skills, and gain greater control of their lives.

Call our Referrals Team or complete the online form. We aim to respond to all referrals within 24 hours and offer accelerated assessments for urgent placements.

Assessing needs – We’ll arrange a one-to-one assessment to spend time with the person, their family, and others that are important to them. We will get to know their current situation, support and accommodation needs, and ambitions.

Explore options – Where appropriate, we will arrange for you to visit the recommended service, meet the team and other people we support, and ask questions.

Personalised support plan – We’ll produce a support proposal detailing the arrangements and the support hours required. We’ll keep you up-to-date every step of the way.
Transition support – Once the funding is agreed, we’ll agree on a start date and begin the transition arrangements. Our local team will ensure everything runs smoothly.
