About Chosen Care Supported Living

Chosen Care Supported Living is a supported living and domiciliary care service providing personal care and support to adults with a variety of needs, including learning disabilities, autistic spectrum disorder, and mental health needs. We currently have 12 supported living services and 4 outreach.

Our supported living services provide high-quality, person-centred support, and enablement for adults with disabilities and support needs. Chosen Care’s pathway is to ensure continuity of care and locality to our individual people with support needs and aspirations; ensuring their goals and choices are met.

Our teams are trained to specialise in the needs of all people we support, such as PMLD, use of different aids hoist, acquired brain injuries, and complex behaviours that may challenge as well as autism, mental health, and wellbeing.
